Application/Certification Material for Laboratories
This page lists the materials your laboratory must submit in order to apply for certification to perform testing.
The required forms are provided on this page.
Instructions
To apply for Secondary NELAP accreditation in New York, submit the following:
- Form 107
- Secondary category forms as appropriate: Form 108, 109, 1977 and/or 1978
- Personnel credentials as indicated in Item 140 of the Certification Manual
- Primary Accrediting Body’s (AB) NELAP Certificate(s)
- Your lab's quality manual
- Most recent assessment report and corrective action response performed by Primary AB
- Secondary does not apply to Asbestos and Fibers in Air and Emissions, Asbestos in Friable and Non-Friable Materials, Legionella, Critical Agents, Lead in Paint, Lead in Dust Wipes, Radon in Air, and Total Microcystins.
To apply for Primary NELAP or ELAP (for Asbestos, Lead in Paint, Lead in Dust Wipes, Legionella, Total Microcystins, Critical Agents and Radon) accreditation in New York, submit the following:
- Form 107
- Primary category forms as appropriate: Form 108, 109, 1977, 1977CA and/or 1978
- Personnel credentials as indicated in Item 140 of the Certification Manual
- Your laboratory’s quality manual
- Your laboratory’s standard operating procedures
- Your laboratory’s demonstration of capability forms
- Documentation of successful participation in proficiency testing
Please be aware that an inspection by an ELAP Consultant may also be required prior to certification.
